Confirmation of Origin and Review of Policies and Procedures
in the Academic Policies and Procedures Manual (APPM)

In order to confirm that policies and procedures in this manual have had proper origin and review, the Academic Policies
and Procedures Manual Advisory Committee has specified the following as being appropriate:
- If a policy or procedure is the result of the action of an appropriate
committee, council, or office within the university, then the identity of that
body or office should be indicated in the APPM, along with the date of the
establishment of that policy or procedure.
- If a policy or procedure is mandated by an outside agency, then an appropriate
administrative office should certify the source of the requirement (e.g.,
law or board regulation) and that source should be indicated in the APPM,
along with the date of the establishment of that policy or procedure (if
known).
- If a procedure has been established by a body or office to implement (not
to establish) policy that was established by a different body, office, or
agency in (a) or (b) above, then the APPM should indicate the body or office
that established the policy, along with the date of its initiation, as well
as the body or office that established the implementing procedure, along
with the date of establishing that procedure.
- A change in the language of an item that has been authorized by the
Academic Policies and Procedures Manual Advisory Committee for inclusion in
the Academic Policies and Procedures Manual may be proposed by the same body
that approved the item. If the Committee determines that the proposed change
is within the purview of that body, the change will be entered into the APPM.
If the Committee determines that the change affects policy or procedure beyond
the purview of that body, then it will be referred back to the proposer for
consultation with other appropriate bodies. Purely editorial changes
(e.g., correction of typographical errors) may be effected by the Committee
chair and the word-processing staff. (April 28, 1994)
